Discharge from Whipps Cross University Hospital

Why hospital discharge services are important

Being discharged from hospital, returning home and settling backing can be a difficult time for an older adult, especially for those who live alone. Age Concern Waltham Forest provides a number of services that offer support and extra security during discharge or on return home.

These aim to:

  • ensure a safe and secure discharge
  • address any concerns or anxieties that you may have
  • answer any questions that you have - or get the answers for you
  • provide practical help such as shopping and pension collection
  • assist you identify any services that you may need or want
  • offer you a benefit check
  • give you information on a range of issues

You will find Age Concern Waltham Forest staff in Whipps Cross University Hospital - in Wards and in the Discharge Lounge. They will approach you and will be wearing an identity badge. You can also speak to Nursing staff in the Discharge Lounge to verify their identity or ask a hospital staff member to call Age Concern Waltham Forest on 0208 558 8716. Age Concern Waltham Forest staff will talk with you about your discharge and any concerns that you may have. They will ask you to complete a questionnaire that will ensure that you are referred to any services that you may need and/or that you receive appropriate information.

Welcome Home

The Welcome Home Service provides assistance and support to older people on discharge from Whipps Cross University Hospital. Staff are available to visit older people on the day of the discharge to assist with practical tasks. Staff can assist with shopping, collection of pension and prescription and other practical tasks, such as small household tasks. Referrals are accepted from Social Workers, Occupational Therapists, Ward Staff, Consultants, older people and Age Concern Waltham Forest staff. Referrals should be made on the day prior to discharge on 0208 558 8716.

Benefits Checks

This service is for older patients who have difficulties understanding welfare benefits, who have a change in their circumstances, or who would like their benefits checked to ensure that they are getting all that they entitled to. We can assist also with form filling.

We provide confidential advice and information on other issues such as community care assessment, long-term-care, or complaints about service.

We accept referrals from Whipps Cross Hospital Social Workers as well as hospital staff. Any older patient can also refer themselves to the service.

Information and Advice can be provided in most of the Asian languages spoken in Waltham Forest.

Referral can be made by telephone and in writing to the manager of the service.

Escorted Discharge from Accident and Emergency (A&E)

This service provides an escorted discharge to a patient who would be going home with relatives or friends, if those were available. Staff will settle the patient into their home surroundings and provide shopping, pension collection and minor housework tasks.

Referrals are accepted from the Minors Department, Reception Desk, OT's and Consultants in the Aamp;E department. The coordinator will talk to you about your discharge and any concerns you may have going home and will provide you with information to ensure your wellbeing within the community.

Referrals can be made direct to the Coordinator who is based at the Whipps Cross University Discharge Lounge and can be contacted on 07793394117 or on 0208 535 0050 Extension 5109.
